Diabetes for those with IDD

During each visit Members/Families should confirm that your care manager is taking the following into consideration:


Have there been any labs, tests or provider appointments since the prior visit?

Follow up activities could include:

-Document details regarding the labs, tests, provider visits and medication changes since the prior visit.

-Identify if it has been over 6 months since the member was last seen by their provider.

-Identify any barriers and assist the member with making these arrangements.

 

Has the member been checking their blood sugars as ordered by the provider since the last visit?

Follow up activities could include:

-Identify if member is following treatment plan. If the member/caregiver is unable to identify how often to test their blood sugars- the care manager will assist the member to obtain this

information from the provider’s office.

 

Does the member has the supplies needed to test their blood sugars?

Follow up activities could include:

-Identify any barriers.

Identify the need to assist the member to obtain necessary supplies.

 

Does the member/caregiver know what their blood sugar numbers should be and when to contact

their provider’s office (too high/too low/for what numbers)?

Follow up activities could include:

-Identify member’s understanding of when to contact the provider.

-Identify the need to assist this member to obtain this information from the provider.

 

Has the member had their weight checked since the last visit?

Follow up activities could include:

-Identify if the member’s weight is of a concern based on member’s current BMI (please see

link below for BMI calculator).

-If the weight is of a concern, identify the member’s interest in assistance with weight

loss and provide information/resources from the resource page.

 

Is there a change in the member’s activity level since the last visit?

Follow up activities could include:

-Identify any changes in activity level that may impact member’s weight and/or cholesterol.

-Identify member’s interest in increasing activity level and if interested provide information/resources from the resource page.

 

What is the member’s doctor has suggested regarding diet and activity?

Follow up activities could include:

-Identify member’s interest in a diabetes education program.

-Programs in the area can be identified by checking with their local hospital and/or going onto the following site: https://www.diabetes.org/diabetes/type-2/living- with-type-2-diabetes-program

 

When was the member’s last eye exam?

Follow up activities could include:

-Identify if the exam has been within the last year.

-Identify any barriers if the exam was longer than a year ago.

-Identify the need to assist member to make arrangements.


Does
the member see a podiatrist (foot doctor)?

Follow up activities could include:

-If yes- identify the date of the last exam.

-Identify if it has been longer than 3-6 months.

-Identify any barriers.

-Identify need to assist with arrangements.

-If no- identify who is cutting the member’s toenails (high risk for infection with any cuts/open

areas).

-Identify need to encourage podiatry care.

-Identify need to assist with arrangements.

 

Is the member interested in any educational information?

Follow up activities could include:

-Identify the member’s interest.

-Identify what information/resources to provide the member from the resource page.
